That many people with gender dysphoria describe trouble concentrating is not just a subjective impression. A new study suggests that this distress may be physically «wired» through a specific brain circuit, and places the key piece in the thalamus and its connection with the prefrontal cortex.
Researchers from several Chinese universities analysed by resting-state functional magnetic resonance imaging (fMRI) 44 transgender women, 40 cisgender men and 42 cisgender women. They measured the strength of the functional connections between the thalamus —a deep brain station involved in attention and in the filtering of information— and different regions of the cortex. They also recorded the participants’ gender dysphoria symptoms and their cognitive performance, with particular attention to attention and vigilance.
The results show that transgender women display shared and distinctive traits. Like cisgender men, they showed increased connectivity in several circuits between the thalamus and the prefrontal cortex. But, unlike them, they presented a weakened connection between medial regions of the thalamus and the left dorsolateral prefrontal cortex.
The central finding is a different one. In transgender women, the intensity of dysphoria symptoms was associated with the strength of the connection between the thalamus and the medial prefrontal cortex, and that same pathway was related to attention. The statistical analyses revealed that this connection acts as a bridge: it explains the relationship between the distress of gender incongruence and problems of attention and vigilance. Put another way, gender dysphoria does not simply coexist with cognitive difficulties; its link appears to run through an identifiable brain circuit.
The authors consider that they have identified a novel neural pathway connecting the symptoms of gender dysphoria with cognitive function. They stress, however, the exploratory nature of the work: a single-centre sample, a retrospective design and a limited size that calls for confirmation in larger and more diverse cohorts before firm conclusions can be drawn.
Understanding these mechanisms is not intended to label the brain as «different», but to contribute knowledge about the biological bases of gender dysphoria and, in the future, to guide better-tailored support for those who live with it.
